Chapter 1 - The Ghost in the Doorway

The air in the small clinic room fractured, shattering into a million jagged pieces the moment the second man stepped through the door. Mara’s breath seized in her throat, trapped behind a sudden, insurmountable wall of terror. She stared at the man standing just inside the threshold. He was drenched, his dark canvas jacket heavy with the cold Tuesday rain, his chest heaving with violent, ragged gasps. His face was a perfect, flawless mirror of the man who had just threatened her life. The sharp angle of his jaw, the dark, unkempt waves of his hair, the exhausted hollows beneath his gray eyes—they were identical.

But there, resting just above the frayed collar of his damp shirt, was the pale, coin-sized burn scar. The true mark of the man she loved.

The profound, agonizing realization of what this meant crashed over Mara with the crushing weight of a collapsing building. For months, she had been sharing her home, her table, and her bed with a phantom. A predator wearing her husband's face. The days of cruelty, the sudden left-handedness, the terrifying, hollow look in his eyes—it had never been Evan losing his mind. It had been a stranger violating the sanctity of her life, studying her, mimicking her husband while the real Evan was away breaking his back in Milltown. The psychological weight of the betrayal was a physical agony, sharper and more devastating than the severe anemia draining the blood from her veins.

"Who just walked out of here?" the scarred Evan demanded, his voice tearing at the edges, thick with a frantic, desperate panic. He took a heavy step toward the bed, his gray eyes locking onto Mara’s pale face. "Mara, please. Tell me he didn't touch you."

Nurse Ruth Bennett did not hesitate. Her sixty-one years of life had been spent navigating the brutal, unpredictable underbelly of the city’s charity wards. She possessed a visceral understanding of danger. She stepped squarely between Evan and the hospital bed, her broad shoulders forming an impenetrable physical barricade.

"Do not take another step toward this bed," Ruth barked, her voice carrying the absolute, uncompromising authority of a seasoned veteran. She reached behind her, her hand hovering protectively near Mara’s trembling shoulder.

Dr. Hannah Cole moved with equal, terrifying efficiency. She snatched the heavy black receiver from the wall-mounted telephone, her eyes fixed on the man in the doorway. "Security," she snapped into the receiver, her tone clipped and devoid of any panic. "Initiate a full lockdown of the maternity wing. We have a hostile intruder on the premises. Black hair, dark work jacket, indistinguishable from the man currently in Room 3. Seal the exits."

Evan ignored the doctors. He didn't care about the lockdown or the hospital protocols. His entire world was currently bleeding on a sterile mattress. He dropped to his knees right where he stood, the dirty, rain-slicked linoleum soaking through the denim of his jeans. He looked at Mara, the sheer, unadulterated terror in his expression breaking her heart.

"It's Julian," Evan whispered, the name sounding like a curse, a venomous thing he was forced to spit out. "I thought he was dead, Mara. I swear to you on my life, I thought he burned in the same fire that gave me this scar."

"Julian?" Mara choked out, her hands instinctively wrapping protectively around her swollen belly. The baby kicked violently against her palms, sensing the massive spike of adrenaline flooding its mother's system. "Who is Julian?"

"My twin brother," Evan said, his voice breaking, tears mingling with the rain on his cheeks. "He found me three months ago. He’s been watching us. He’s been hunting me."
