Chapter 2 - The Debt of Blood

The clinic room fell into a heavy, suffocating silence, punctuated only by the rapid, frantic beeping of the fetal heart monitor tethered to Mara’s side. The revelation hung in the air, toxic and heavy. A twin brother. A secret kept hidden in the dark corners of Evan’s past, now violently resurrecting itself to destroy their future.

"Why didn't you tell me?" Mara demanded, her voice gaining a sudden, desperate strength fueled by a rising, protective fury. The image of the "other Evan" pressing his ear to her stomach, pretending to love her child, made bile rise in the back of her throat. "He was in our house, Evan! He slept next to me! How could you not know?"

"Because I was working double shifts at the Milltown foundry to pay for the hospital bills we knew were coming!" Evan pleaded, his hands gripping the edge of the metal bedframe, though Ruth immediately swatted them away. He looked up at Dr. Cole, then back to his wife. "Julian is a ghost. He’s a chameleon. Ten years ago, he got deeply, violently indebted to the Calloway Syndicate—a mafia outfit running the northern docks. He stole two hundred thousand dollars from their offshore accounts and vanished. They burned our family home to the ground looking for him. That's how I got this scar."

Dr. Cole slowly lowered the phone receiver, her clinical mind rapidly processing the sheer, dangerous magnitude of the situation. "And now he’s back. Why?"

"Because the Calloways finally found his trail," Evan explained, his words rushing out in a frantic, terrified torrent. "He came to me three months ago, begging for money. I told him I had nothing. I told him I was having a baby. I refused to let his poison touch my family."

Evan swallowed hard, looking at the bruised, needle-pricked veins on Mara’s arm. "I didn't know he was stepping into my life when I was gone. I didn't know he was studying me. He’s been systematically draining our savings accounts, intercepting our mail, and… God, Mara, your anemia."

Mara’s breath hitched. She looked at Dr. Cole.

"Julian isn't just hiding," Evan whispered, the horrific truth finally breaking through the surface. "He’s trying to steal my identity permanently. To do that, he needs the real Evan Reed to disappear. But I have a wife. A wife who is about to have a baby. A baby that would complicate a clean getaway."

Dr. Cole’s face went entirely pale. She looked down at Mara’s chart. "The extreme blood loss. The constant fainting. It isn't just poor nutrition, Mara. He’s been quietly, methodically poisoning your food. He was trying to induce a miscarriage. He was trying to erase your child so he could erase your husband without any loose ends."

The sheer, staggering magnitude of the malice struck Mara like a physical blow. She wasn't just a victim of a bizarre psychological game. She was a target in a high-stakes, violent mafia cover-up. Julian was a desperate, cornered rat, willing to murder an unborn child and a helpless woman just to wear his brother's skin and escape the wrath of the Calloway Syndicate.

Suddenly, the overhead fluorescent lights flickered, hummed violently, and then died completely. The room plunged into darkness, save for the faint, eerie glow of the battery-powered fetal monitor.

"He cut the main breakers," Ruth said, her voice dropping to a low, dangerous growl. "The backup generators take ninety seconds to kick in."

"He’s not running," Evan said, pushing himself up from the floor, his gray eyes adjusting to the darkness, filled with a sudden, lethal resolve. "He knows the lockdown is in effect. He knows he can't get out of the building undetected. He’s coming back to finish the job."
